DIGS Merchandise - 2009 - ORDER BY FAX
INSTRUCTIONS
Below is the order form for DIGS Gears 2009. We are
offering a hooded sweatshirt, sweatpants, a fleece jacket, and a long sleeve
T-shirt, all with DIGS logo.
The hooded sweatshirt comes printed on the back with the name of the person listed on the form . If you do not want the name included (say, if the sweatshirt is for a parent), write "no name" next to the item on the form.
Orders by fax will be accepted through Friday, March 6, 2009. Orders received after that time cannot be processed.
Please complete and fax the order form below. Orders should be faxed to the number below (attention: Charlotte Castillo) by March 6 and a check for the total amount mailed immediately to the DIGS P.O. Box below.
