Four-step Process to Register for Clinic / Team Try-outs
Step 4: Mail In Forms And Documents
Please assemble into a single packet and mail to DIGS the following:
- The two signed GEVA forms you printed from Step 2:
- GEVA Registration
- USA Volleyball Medical Release.
- One copy of the DIGS registration confirmation page you printed from Step 3.
- One copy of your proof of age (either birth certificate or passport).
- One copy of your health insurance identification card (both front and back of card!).
The proof of age and medical insurance card information are critical. You cannot participate in clinic without submitting them in advance.
Packets received without online payment completed will not be accepted.
For Sophomores and Older - Clinic starting Sunday,
November 12 - DO NOT MAIL:
Bring into clinic on Sunday.
